Rest assured your private information will be held in the strictest confidence and used only to provide excellent personal training services. And there is no judgement here – please feel free to be completely honest.
If you would prefer a paper copy, please let me know and we can complete this together at your first session.
Measurements and your initial interval photos will be taken during one of your first few sessions, and will be updated every six months. You will have access to these through your training app, which I will invite you to join once I have received this form.
